Admission Forms

Below, you will find the admission forms for Ironwood. All of the forms are in PDF format. If you do not have a PDF viewer, you can download one for free by clicking here. The pre-admission form is also available in Word format, so it can be completed for e-mailing to Ironwood.

In addition to the above-mentioned forms, supporting documentation is also required. In order to process your child's application in the most timely manner, the admissions committee will also need copies of educational records (transcripts, report cards, educational evaluations, etc.), medical records (immunization records and prescriptions for any medications currently being administered), any clinical records (psychological evaluations, therapists notes, discharge summaries, progress notes, etc.),and legal documents (custody orders or other pertinent legal documents). These documents can also be faxed or mailed.

Immunizations

For required immunizations needed before your child enters the Ironwood program, we suggest you comply with the CDC guidelines as instructed by your primary care provider. Please discuss the need for a tetanus booster and the meningococcal vaccine.

Clothing for Ironwood

Please provide the following clothing items for your child upon arrival to Ironwood. If you don’t already have these items we suggest you shop at www.llbean.com or comparable. Please do not send items with visible brand names or logos. Ironwood will provide t-shirts and sweatshirts.
